Mariah’s Miracle Financial Assistance
Mariah’s Miracle proudly supports Arizona pediatric therapy patients and their families by helping cover the cost of therapy and equipment not provided by insurance. Our assistance is determined on a case-by-case basis, depending on available funds and eligibility criteria.
Eligibility Criteria
To qualify for assistance, the applying family (parent/legal guardian) must:
Reside in Arizona.
Have a child under 17 receiving physical, occupational, or speech therapy.
Provide medical documentation justifying the child’s need.
Show proof of insurance denial or a lack of coverage.

Individualized Review: Applications are reviewed individually, prioritizing first-time applicants.
Information Verification: Applications with false information will be denied.
Fair Evaluation: Determinations are made without regard to race, religion, or gender, based solely on available funds.
Approval Details: Approved applications will outline covered equipment or the duration of therapy assistance.
Direct Payment: Payments are made directly to the therapy or equipment provider.
Reapplication Opportunities: Families may reapply after 3 months or if a child’s health status changes significantly.
Flexibility: The Mariah’s Miracle Board of Directors may adjust guidelines when necessary.
